[Gastric cancer 709 - 주름은 넓은 위암의 한 부분으로 converging 할 수 있습니다.]

작고 깊은 궤양형 암으로 의뢰되었습니다 (좌측 사진). 내시경 재검을 하였는데 영 모양이 달랐습니다 (우측 사진). Fold가 끌려오는 궤양 부위가 없었던 것은 아니지만 주변의 넓은 변색 (주로는 탈색, 일부 발적) 부위가 보였습니다. 처음에는 EGC IIc 혹은 EGC III 였으며 넓은 변색 부위를 발견하지 못하였는데 위암 발견 후 약을 드시면서 의뢰되었으므로 모양이 바뀌어 EGC IIb + IIc처럼 변경되었고 넓은 변색부위 중앙의 함몰부까지 주름이 당겨진 모양이 된 것입니다. 즉 주름은 EGC IIb 부위에서 중단되지 않고 함몰부까지 끌려오는 모습입니다.

수술 후 병리입니다.

10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Associated findings : gastritis cystica profunda
13. AJCC stage by 8th edition: pT1b N0

Text로 된 결과만 보면 느낌이 오지 않습니다. 이럴 때에는 gross를 살피면 도움이 됩니다. 5cm였다는 것을 금방 수긍할 수 있습니다.

주름은 넓은 위암의 한 부분으로 converging 할 수 있습니다. 함몰형 위암이 발견되면 주변 점막의 변화를 잘 살펴보면 좋습니다.
