조직검사에서 Poorly differentiated adenocarcinoma with signet ring cell carcinoma component로 나와 의뢰된 환자입니다. 여러분은 ESD를 하시겠습니까?

저는 수술을 권하였고 아래의 결과였습니다.

Stomach, subtotal gastrectomy: Early gastric carcinoma
1. Location : lower third, Center at antrum and posterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, poorly (solid) differentiated
4. Histologic type by Lauren : diffuse
5. Size : 2x2 cm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 5 cm, distal 2 cm
8. Lymph node metastasis : no metastasis in 22 regional lymph nodes (pN0) (0/22: "3", 0/6; "4", 0/5; "5", 0/1; "6", 0/0; "7", 0/3; "9", 0/0; "8a", 0/2; "11p", 0/3; "12a", 0/2; "4sb", 0/0; "1", 0/0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 8th edition: pT1a N0

언젠가는 적응증을 확대할 수 있겠지만, 아직은 합의된 의견에 따라 치료하고 있습니다. Poorly differentiated는 수술, signet ring cell carcinoma라도 1cm가 넘거나 경계가 불분명하면 수술을 하고 있습니다. 그러니까 undifferentiated type histology를 가진 조기위암에서 저는 1cm 이하의 경계가 명료한 flat한 SRC만 ESD를 하고 있는 셈입니다.
