[Gastric cancer 759. AGC with common hepatic node metastasis]

위 주변 림프절 중 common hepatic node는 참으로 애매합니다. Anterior common hepatic node (#8a)는 group 2이고 posterior common hepatic node (#8p)는 group 3입니다. Group 3는 과거 방식으로는 metastasis로 간주되는 영역입니다.

진행위암인데 CT에서 common hepatic node가 현저히 커져 보였습니다. CT로 대강은 알 수 있으나 anterior인지 posterior인지 정확히 알기는 어렵습니다. 수술해도 약간 마찬가지인 면도 있으나... 외과 의사와 상의하였고 수술을 진행하였습니다

1. Location : lower third, Center at antrum and lesser curvature
2. Gross type : Borrmann type 2
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : mixed
5. Size : 5.5x4.6 cm
6. Depth of invasion : penetrates subserosal connective tissue (pT3)
7. Resection margin: free from carcinoma, safety margin: proximal 4 cm, distal 3 cm
8. Lymph node metastasis : metastasis to 13 out of 47 regional lymph nodes (pN3a) (perinodal extension: present) (13/47: "3", 8/16; "4", 2/8; "5", 2/3; "6", 0/6; "7", 0/1; "9", 0/1; "8a", 1/4; "11p", 0/1; "12a", 0/1; "4sb", 0/3; "1", 0/1; "12p", 0/2)
9. Lymphatic invasion : present
