Multiplicity of early gastric cancer is a well-known. Sometimes, another cancer is found in the first follow-up endoscopy after a successful ESD for EGC. Of course, the patient is not happy. The doctor is unhappy, too.

ESD was done as usual for the second cancer. The procedure was difficult due to the location. A big loop approach was used.

ESD: Early gastric carcinoma
1. Location : body
2. Gross type : EGC type IIa
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 16 mm (2) vertical diameter, 10 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 12 mm, proximal 10 mm, anterior 14 mm, posterior 6 mm, deep 400 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

When another cancer was found after ESD within a year, we call it a synchronous cancer.

오늘 증례와 같이 첫 내시경에서 발견되는 synchronous cancer를 줄이기 위해서는 첫 내시경 검사 시 위를 구석구석 잘 살펴야 합니다. 암 병소가 하나 발견되었다고 기뻐하기보다는 또 다른 병소가 없는지 다른 부위도 자세히 관찰하는 여유가 필요합니다.
ESD 시술자의 입장에서는 외부에서 암이 발견되어 의뢰되었을 때 시술 전 재검을 할 필요가 있는가 고민입니다. 시술 전 재검을 한다면 synchronous cancer 발견은 줄일 수 있을 것입니다. 그러나 반복 검사로 인하여 환자의 고통이 상당할 것이고, 조직검사 재검으로 인한 혼선이 상당할 것이고, 재차 시행되는 조직검사로 인한 fibrosis가 문제일 것입니다. 조직검사 재검은 하지 않고 그냥 관찰만 하는 전략도 가능한데 이 또한 몇 가지 논리적인 어려움이 있습니다. 그래서 저는 외부의 첫 내시경 사진이 비교적 깨끗하면 시술 전 내시경 재검을 하지 않고 있습니다.^^
