A small ulcer was found during the screening endoscopy for 50 years old male. The endoscopy report was "One small deep ulcer (on MB GC). Biopsy 이상 없어도 대학병원 소화기 내과에서 반드시 추적검사 하세요."

After referral, the image was interpreted as AGC Borrmann type 4. Surgery was done

작은 병소의 조직검사에서 암이 나왔고 radical total gastrectomy가 시행되었습니다. 한 가지 아쉬운 것은 첫 진단의 impression이 gastric ulcer였다는 것입니다. 후향적으로 살펴보면 보만 4형의 전형적 소견인데 말입니다.

위내시경 검사의 첫 단계는 "보만 4형 진행성 위암은 아닌가?" 살피는 것입니다. 보만 4형이 아니면 다음 단계는 focal lesion은 없는지 구석구석 살피십시요. 만약 focal lesion이 발견되면 주변 점막은 문제가 없는지 검토해야 합니다. 그러면 처음에 보만 4형을 의심하지 못하였더라도 focal lesion 주변을 보면서 "아하! 보만 4형이구나" 깨닫게 됩니다.
