중년 남성이 "GC of LB, 1.5cm, EGC IIb, 조직검사: well differentiated tubular adenocarcinoma"로 의뢰되셨습니다. 의뢰서 내용만 보면 ESD를 시행하면 적당한 환자입니다. 그러나 경계가 불분명하고 함몰 변색형이고 뚜렷하게 주름이 끊긴 형태임으로 저는 수술을 권하였습니다.

초기 검사를 시행하였습니다. 외부 슬라이드 재판독에서는 위암 조직형의 관찰자간 차이가 있어 M/D로 보고되었습니다. 내시경 재검과 조직검사 재검 소견은 아래와 같았습니다. 헬리코박터 항체는 양성 (119.8), pepsinogen 1 : 52.7, pepsinogen 2 : 24.6, pepsinogen 1/2 ratio: 2.1이었습니다.

CT에서 병소는 보이지 않았고 림프절 전이는 없었습니다.

수술을 시행하였습니다.

7. Resection margin: free from carcinoma, safety margin: proximal 1.5 cm, distal 10.5 cm
8. Lymph node metastasis : no metastasis in 33 regional lymph nodes (pN0) (0/33: "3", 0/7; "4", 0/7; "1", 0/4; "11p", 0/1; "12a", 0/4; "4sb", 0/0; "5", 0/0; "6", 0/2; "7", 0/2; "8a", 0/3; "9", 0/2; "4d", 0/1)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 8th edition: pT1a N0

Tubular adenoarcinoma, moderately differentiated

병리결과를 보고 다시 생각하면 넓은 의미의 ESD expanded indication에 해당한다고 볼 수 있습니다. 그러나 이처럼 뚜렷한 함몰형이고 주름이 끊어진 현상이 현저하고 크기가 4cm 정도면 현실적으로 ESD를 권하기는 어렵습니다. 아마 다른 ESD specialist들도 마찬가지 아닐까 싶습니다.

이 환자는 수술 전 혈액검사 결과에서 헬리코박터 양성이었고 pepsinogen 1/2 ratio가 3이하로서 혈청학적 위축성 위염 상태였습니다.
