M/D adenocarcinoma라는 조직검사 결과 의거 ESD를 하였습니다.

병리 결과에 M/D adenocarcinoma with mucin production (20%)라는 부분이 있어서 병리과 선생님과 함께 슬라이드 검토를 했습니다. Gland를 형성하는 분화조직형이고 점막에 국한된 암이 틀림없는데 국소적으로 mucin pool이 있었습니다 (아래 사진 검은 점선).

위암의 아형 중 mucinous adenocarcinoma라는 것이 있습니다. 그런데 mucin pool이 50%가 넘는 경우에 mucinous adenocarcinoma라고 붙인다고 합니다. 50%가 되지 않으면 병리 결과에는 언급을 하지만 mucinous adenocarcinoma라고 명명하지는 않습니다.

WHO classification of tumors (5th edition, 2019)에서 mucinous adenocarcinoma 부분을 옮깁니다. Gland를 형성하는 것도 있고 그렇지 않는 것도 있다는 이야기입니다.

"Two main growth patterns can be seen. (1) recognizable glandular structures or tubules line by columnar epithelium with intestinal mucin and (2) chains, nests, or single tumor cells (signet-ring cells can be seen) surrounded by mucin."
