경계가 약간 불분명하고 융기부 표면이 울퉁불퉁하면서 중앙의 함몰부가 비교적 명확히 보여서 (이 사진에서는 다소 비스듬히 찍혔습니다만) 수술을 권했습니다. 위암의 심달도 진단은 항상 어렵습니다.
Early gastric carcinoma
1. Location : middle third, Center at body and lesser curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 2.3x1.5 cm
6. Depth of invasion : invades submucosa (sm2) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 2.5 cm, distal 13 cm
8. Lymph node metastasis : no metastasis in 32 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : present (intramural)
11. Perineural invasion : not identified
12. Peritoneal cytology : negative

다시 봐도 어렵습니다. 쉬운 위치의 병소의 경우는 diagnostic ESD 개념을 적용하여 비수술적 치료를 고려할 수 있지만, 어려운 위치의 병소는 indication 기준을 tight하게 적용할 수 밖에 없습니다.
