[2016-9-19. 애독자 질문]
2년 전 보만 4형 진행성위암으로 위전절제술 받으신 분으로, 최근 dysphagia 소견으로 내시경 검사를 시행하였는데 anastomosis site 바로 아래 부위로 종괴가 발견되어, recurred gastric cancer로 판단하였습니다. 궁금한 것은,
1) total gastrectomy 를 받으신 분인데 recurr 된 mass 의 병변을 stomach 으로 보는 것이 좋은지, 아니면 small intestine 으로 보는게 맞을지 궁금합니다.
2) 진단명으로 r/o recurred gastric cancer 로 하는 것이 나을지, 아니면 metastatic cancer나 다른 병명으로 하는 것이 좋을 지 고민입니다.

사진을 검토하였습니다. 첫 내시경 하신 선생님께서 함몰부를 발견하여 위암을 진단하셨습니다. 당시에는 아마 주변의 광범위한 fold 변화에 큰 관심을 주시지 못하셨을 수도 있을 것 갔습니다. 여하는 재검에서 명백한 보만 4형 진행성 위암이었고 수술을 받으셨는데 안타깝게도 재발한 경우입니다. CT에서 peritoneal seeding도 있었습니다.
Advanced gastric carcinoma
1. Location : upper third, Center at high body and anterior wall
2. Gross type : Borrmann type 4
3. Histologic type : tubular adenocarcinoma, poorly differentiated
4. Histologic type by Lauren : diffuse
5. Size : 7x6.5 cm
6. Depth of invasion : penetrates subserosal connective tissue (pT3)
7. Resection margin: free from carcinoma, safety margin: proximal 2.3 cm, distal 8.1 cm
8. Lymph node metastasis : metastasis to 2 out of 45 regional lymph nodes (pN1) (perinodal extension: absent) (2/45: "1", 0/0; "2", 0/7; "3", 2/12; "4", 0/8; "4sb", 0/2; "5", 0/0; "6", 0/5; "8a", 0/0; "7", 0/5; "9", 0/1; "11p", 0/3; "12a", 0/2)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : negative
13. AJCC stage by 7th edition: pT3 N1답변 1) Anastomosis site에 연하여 직하방 jejunum에서 재발소견이 보였고 조직검사에서 암이 확인되었습니다. 따라서 병리 의뢰시 장기명(organ name)은 small bowel입니다. 폐암이 위 전이를 보여 위내시경으로 조직검사를 하였을 때 장기를 lung으로 쓰지 않는 것과 마찬가지입니다.
답변 2) 보만 4형 진행성 위암 재발은 대부분 local recur와 peritoneal seeding이 함께 있습니다. 이 경우도 마찬가지였습니다. 내시경 후 진단명은 Local recurrence just below the anastomosis site로 쓰십시오.
