좌측: 의뢰 전 내시경, 우측: 의뢰 후 내시경 재검
보만 type이 다소 모호했는데요... 전형적인 B-II가 아니면 B-III로 붙일 것을 권하고 있습니다. 내시경을 배우고 있는 전공의 선생님께서 이 환자의 소견을 아래와 같이 기술하였습니다. 아주 잘 하셨습니다.
"AW-GC of proximal antrum에 round shape의 중앙에 함몰부를 동반한 elevated lesion이 관찰된다. 중앙 함몰부는 edge는 뚜렷하지 않으며 base는 옅은 exudate로 덮여있다. Margin은 elevation 되어 있다. Angle에서 부터 converging fold가 끌려오며 주변으로부터 bridging fold가 형성되어 있다. 함몰부 주변의 mucosa는 광택이 있으며 매끈하다. Impression: AGC Borrmann type III"
병리 결과입니다.
Stomach, radical subtotal gastrectomy: Advanced gastric carcinoma
1. Location : lower third, Center at antrum and greater curvature
2. Gross type : Borrmann type 3
3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : diffuse
5. Size : 3.3x3.1 cm
6. Depth of invasion : invades muscularis propria (pT2)
7. Resection margin: free from carcinoma, safety margin: proximal 4.5 cm, distal 4.8 cm
8. Lymph node metastasis : metastasis to 2 out of 40 regional lymph nodes (pN1) (perinodal extension: absent) (2/40: "3", 0/7; "4", 1/5; "5", 1/5; "6", 0/0; "7", 0/3; "9", 0/1; "8a", 0/4; "11p", 0/0; "12a", 0/6; "4sb", 0/4; "1", 0/4; "12b", 0/1)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
