[Gastric cancer 692 - Metachronous after ESD for EGC]

전정부 대만 위암 ESD 후 재발 없이 잘 지내던 환자가 8년만에 far distal antrum의 metachronous lesion으로 오셨습니다. 그런데 AGC였습니다. Stent 때문에 몇 년간 검사를 받지 못하다고 최근 복통이 있어 검사를 하셨다고 말씀하셨습니다.

관상동맥 stent 환자는 aspirin과 clopidogrel 혹은 유사 약제를 드시기 마련입니다. Stent 후 항혈소판제를 끊지 말라는 가이드라인은 있어도 내시경 검사를 하지 말라는 가이드라인은 없습니다. 내시경 검사 전 aspirin과 clopidogrel은 끊어야 하는 약도 아닙니다. 관상동맥 stent 후라도 매년 내시경을 받아야 하는 환자는 그냥 내시경 검사를 받도록 하는 것이 좋을 것 같습니다. 이런 저런 이유로 내시경 검사를 skip하면 위암이 진행된 상태에서 발견될 수 있기 때문입니다.

세상에는 불필요한 내시경 검사 받는 사람도 많지만 반대도 적지 않습니다. Aspirin, clopidogrel, coronary stent는 내시경 검사의 금기가 아닙니다. 환자를 위험한 곳으로 몰지 맙시다.
