조직검사 결과 “gastric dysplasia, low grade, focal” 소견으로 의뢰된 분입니다. 본 병원에서 외부 슬라이드 재판독을 하였는데 “Adenoma, with high grade dysplasia (Note: The possibility of well differentiated adenocarcinoma cannot be excluded.)”로 나왔습니다. 일단 외래에서 위암으로 진단명을 바꾸었습니다. 환자에게는 아래와 같이 설명하였습니다. 보험회사용 진단서는 치료가 끝난 후 받아가시도록 권했습니다.
"위암으로 진단명을 바꿉니다. 조직검사와 내시경 육안소견 의거입니다. 일단 C code로 바꿉니다만 치료 후 최종 조직검사 결과 의거 암이 아닌 것으로 다시 바뀔 수 있습니다.
ESD를 하였고 역시 예상하였던 결과입니다.
ESD : Early gastric carcinoma
1. Location : cardia, lesser curvature
2. Gross type : EGC type IIb
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 14 mm (2) vertical diameter, 4 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N)
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
병리과 선생님들의 interobserser variation은 매우 큽니다. 병리가 중요한데.. 의학의 기본인데.. 병리가 흔들리면 ESD를 할 수 없는데.. 병리과가 전망이 흐리다고 전공의 지원이 없다는 뉴스를 접하고 있습니다. 참으로 안타까운 일입니다. 우리나라 의료가 뿌리부터 흔들리고 있습니다.
