잔위암의 내시경 절제술은 쉽지 않습니다. 작업할 공간이 부족하기 때문입니다. 그러나 이와 같은 융기형은 상대적으로 수월한 편입니다.

ESD : Early gastric carcinoma
1. Location : high body, lesser curvature
2. Gross type : EGC type I
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 10 mm (2) vertical diameter, 8 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 5 mm, proximal 3 mm, anterior 6 mm, posterior 4 mm, deep 1000 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

맨 마지막 사진을 보시기 바랍니다. ESD specimen을 스트로폼에 고정한 후 촬영을 하였더니 융기형이라는 점이 잘 표현되지 않았습니다. 스트로폼을 거꾸로 뒤집으니 용종형 위암이 천장에 매달린 듯 잘 보였습니다. 내시경 사진촬영의 tip이라고나 할까요.
