내시경에서 매우 큰 암이 있었습니다. 일견 위벽을 넓게 침윤한 것은 맞는데 뚜렷한 융기부도 있어서 내시경적 분류는 Borrmann type 1이 적당하다고 붙여보았습니다.
수술 후 병리결과를 보고 다소 놀랐습니다. 병리과에서는 보만 4형으로 분류하였기 때문입니다.

1. Location : [3] upper third, [1] middle third, [2] lower third Center at body and lesser curvature
2. Gross type : Borrmann type 4
3. Histologic type : tubular adenocarcinoma, poorly differentiated
4. Histologic type by Lauren : mixed
5. Size : 13x9 cm
6. Depth of invasion : invades serosa (pT4a)
7. Resection margin: free from carcinoma, safety margin: proximal 1 cm, distal 1.5 cm
8. Lymph node metastasis : metastasis to 19 out of 50 regional lymph nodes (pN3b) (perinodal extension: present / absent) (19/50: LN3,5, 11/13; LN4,6, 0/5; "1", 0/1; "2", 0/4; "4sb", 0/2; "5", 0/0; "6", 0/4; "7", 1/5; "8a", 1/4; "9", 2/3; "10", 0/0; "11d", 0/2; "11p", 0/1; "12a", 0/2; perigastric LN, 4/4)
9. Lymphatic invasion : present
10. Venous invasion : present(extramural)
11. Perineural invasion : present
12. Peritoneal cytology : positive for malignant cells
13. AJCC stage by 7th edition: T4a N3b
전형적인 보만 4형과는 다른 형태이지만 (1) 뚜렷한 궤양 부위도 없고 (2) 융기부에 비하여 넓게 위벽을 침윤한 부위가 주된 소견이라고 생각되어 보만 4형으로 붙이신 것 같습니다. 내시경으로 볼 때와 절제된 위를 fixation 시킨 후 볼 때에는 제법 다르게 보일 수 있으니까요.
환자는 수술 후 항암치료와 방사선 치료를 받으셨습니다. 비록 나쁜 예후 인자가 많았으나 다행스럽게도 수술 5년째까지 재발이 없었습니다. 좀 더 장기간 경과관찰을 할 예정입니다.
