Recently, various hybrid ESD knives are developed in Korea. I recently performed an ESD using a hybrid ESD knife (Onestep ESD knife) mixing needle-type knife and injecter in a single catheter.

국내에서 개발된 one step ESD knife(제품설명서)를 이용한 ESD 시술을 하였습니다. Injector와 ESD knife를 하나의 catheter로 만든 제품으로 ESD 도중 knife를 넣고 빼고 하지 않고 시술할 수 있어서 매우 유용했습니다. 시간이 매우 절약되었고, assistant의 일이 많이 줄어들어 시술이 compact하게 진행된다는 느낌이었습니다. Tip이 약간 둔하다는 느낌이었은데 이는 두 개의 도구가 좁은 공간에 위치하여 어쩔 수 없는 측면이 있다는 생각이 들었습니다. 국내에서 여러 좋은 도구가 개발되고 있다는 점은 참으로 반가운 일입니다.

3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 8 mm (2) vertical diameter, 7 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N). safety margin : distal 7 mm, proximal 6 mm, anterior 12 mm, posterior 6 mm, deep 200 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Pre-existing adenoma : none
12. Microscopic ulcer : absent
13. Histologic heterogeneity: absent
14. Associated finding: Gastritis cystica superficialis
