위전정부 소만에서 국소 병소가 발견되어 조직검사에서 atypical glands로 나와 의뢰되었습니다. 외부슬라이드 재판독과 내시경 재검을 하였고 결과는 아래와 같습니다.

본 병원 재검: tubular adenocarcinoma, moderately differentiated
외부 첫 내시경: Fujifilm 내시경, 본 병원 재검: Olympus GIF-Q260
그런데 재검을 담당하신 fellow 선생님께서 depressed lesion 두개가 붙어있는 양상이라면서 "옆으로 누워있는 눈사람 모양"이라는 예쁜 표현을 써 주셨습니다. ESD를 하였고 두개의 작은 위암이 나란히 위치한 것으로 나왔습니다. 눈사람이 맞았습니다.

5. Size of carcinoma : ① (1) longest diameter, 16 mm (2) vertical diameter, 10 mm; ② (1) longest diameter, 8 mm (2) vertical diameter, 8 mm
6. Depth of invasion : ①,② invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 8 mm, proximal 13 mm, anterior 14 mm, posterior 10 mm, deep ①:400 ㎛, deep ②: 350 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)

Fujifilm으로 발견되어 의뢰되었고 Olympus로 검사하였고 Pentax로 치료하였습니다.
병리결과에서 WHYX lesion이라 언급된 부분은 일본에서 횡저형위암 (橫這形胃癌, gastric adenocarcinoma of crawling type, 손연결형)이라 부르는 것입니다. Gastric gland가 불규칙하게 fusion하여 마치 알파벳 W, H, Y, X 처럼 보인다는 의미입니다.
