A small flat elevated lesion was found during the screening endoscopy with Fujifilm endoscope. The patient was referred without biopsy. Repeated endoscopy with Olympus endoscope done and the biopsy result was 'atypical gland with high grade dysplasia and focal carcinomatous transformation.

ESD was done with another endoscope, Olympus Dual knife, IT-2 knife, and Finemedix injector needle.

ESD. Early gastric carcinoma
1. Location : antrum, greater curvature
2. Gross type : EGC type IIa
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 20 mm (2) vertical diameter, 15 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 9 mm, proximal 7 mm, anterior 8 mm, posterior 12 mm, deep 150 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

Endoscopies from different vendors have their own characteristics. I enjoy using all the different endoscopies in my daily practice and teaching.

올림푸스는 예나 지금이나 主내시경으로 사용하고 있습니다. 화질이 좋고 내구성이 대단합니다. 내시경계의 왕중왕이라고 할만합니다.
후지필름은 15년 전 쯤 Dr. Yamamoto께서 small balloon enteroscopy를 개발하였을 때 도입하였고, 이후 올림푸스 single balloon enteroscopy로 바꾸었습니다. 현재는 내시경실의 hands-on 교육에 후지필름을 이용하고 있습니다.
