Depth of invasion을 어떻게 추정하십니까?

점막하암으로 생각하고 수술을 보냈습니다. 그런데 PM cancer, 즉 AGC가 나왔습니다. 위암의 침윤깊이 진단은 항상 어렵습니다.
Advanced gastric carcinoma
1. Location : lower third, Center at antrum and lesser curvature
2. Gross type : Borrmann type V
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 2.7x1.5 cm
6. Depth of invasion : invades muscularis propria (pT2) (see note)
7. Resection margin: free from carcinoma: safety margin: proximal 4.2 cm, distal 4.7 cm
8. Lymph node metastasis : no metastasis in 43 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified

EndoTODAY의 Gastric cancers 1000 Season 1을 보다가 질문이 있어서 올립니다. 위암 증례 5/100의 병리 소견 gross type이 Borrmann type V라고 되었는데, 저 정도는 III에 포함되는게 아닌가요? 찾아보니 unclassified 를 V라고 하던데 어떤 경우에 V라고 하는지 궁금합니다.

일단 내시경 사진만으로는 조기위암으로 보는 것이 좋을 것 같습니다. IIa +IIc 형이고 공기를 넣고 빼면서 관찰하니 약간 덩어리진 느낌도 있어서 조금 깊을 수 있겠다 생각하면 됩니다.
병리에서 Borrmann type V라고 주는 것은 사실 좋은 것은 아닙니다. 우리나라 병리의 표준이라고 할 수 있는 위암 병리보고서 기재사항 표준화 (PDF 0.3M)에서는 unclassifiable이라고 쓰게 되어 있거든요 (아래 그림 참조).
우리나라 병리과에서는 보통 육안 소견은 EGC 같은데 현미경 소견은 AGC인 경우 unclassifiable, unclassified, Borrmann type V라고 주고 있습니다.

위암 병리보고서 기재사항 표준화 2판이 2023년 발표되었습니다 (A standardized pathology report for gastric cancer: 2nd edition). 여기에서는 "The unclassifiable type is Borrmann type 5, according to the Japanese guideline"으로 언급되어 있습니다.
